Rather than a still photograph, we look first at expression, overall proportion and vascular anatomy, then set the product, the injection layer and the amount conservatively.

HA filler adds volume where it is placed and is gradually absorbed over time. For smile lines, the doctor often supports the cheek first — restoring volume above the fold can lift and reduce the crease at its source — and then places a softer gel directly along the fold where needed, usually with a cannula in a deeper layer to keep it smooth. Treating the cause and the line together tends to look more natural than overfilling the crease alone. Because this is a mobile area, the fold is softened rather than completely erased. Before you decide, the questions people always ask

  • The groove becomes shallower, but the line itself does not disappear. A smile line sits where the tissue of the cheek has slipped downwards and folded over, so filling the crease alone easily looks unnatural. That is why we first support the flattened part of the cheek and then treat only the groove that still remains.

  • Because the cause sits higher up while only the result is being filled. If the crease is packed while the cheek is still descended, that spot bulges out and looks awkward when you smile. The line softens naturally only when the volume above it is planned at the same time.

  • Arteries that supply the face run close to the smile line, so this area calls for care. We check the depth and the position as we go and place small amounts slowly. If you feel severe pain during the treatment, or the skin feels as though it is blanching, please tell us immediately.

  • This area never stops moving while you talk, eat and smile, so it is absorbed faster than the cheekbones or the temples. How large the difference is depends greatly on the product, the amount placed and how much you speak. If you note how long it took after your first treatment for things to look as they did before, we learn your own absorption rate, and timing the next session becomes easier.

  • When sagging is clearly visible, the natural order is to lift first and then fill whatever groove is left. The other way round, the lifting can push the filler and change its shape. At the consultation we separate how much comes from sagging and how much from lost volume, and then decide the order with you.
