Rather than a still photograph, we look first at expression, overall proportion and vascular anatomy, then set the product, the injection layer and the amount conservatively.
HA filler adds volume where it is placed and is gradually absorbed over time. For the chin, the doctor usually places a firm, high-support gel deep against the bone at the tip (pogonion) to project it forward or downward, and may use a cannula in the layer above to smooth the surface and soften the grooves beside the chin. Because the chin moves less than the lips, a firmer gel here tends to hold its shape and last well. HA can be dissolved with hyaluronidase if an adjustment is needed. Before you decide, the questions people always ask
before consultation
When the chin is short or set back, the lower face can look blunt in profile; bringing that point forward tidies the side line so the face reads slimmer. The difference shows far more from the side than from the front, so we suggest keeping a profile photo beforehand and comparing afterwards.
Projecting the chin forward is not the same as extending it downward. Working too much downward makes the face look longer, so we put the weight on the forward direction and decide while looking at the proportions of the whole face. If a longer-looking chin is what worries you, please tell us at the consultation before we begin.
If the chin muscle tenses often, the filler can shift or the surface can look uneven. In those cases we sometimes consider addressing the strength of the muscle at the same time. If you get a pebbled, dimpled texture on your chin, we check that area together during the examination.
Filler needs no incision, lets you see the result straight away and can be reversed, but it lasts only for a set period. An implant holds for a long time, though it involves surgery and the risks that come with it. We put both side by side against how much change you want and how long you want it to last.
The two are often planned together. Filler handles the length and direction of the chin, while toxin handles the muscle that makes the jaw look angular from the side, so their roles are different. Doing both on the same day makes judgement difficult because of swelling, so it is better to leave an interval and decide the order.
