On the nose, a firmer gel is placed in small amounts along the deep midline — above the cartilage and bone — because the main vessels run to the sides and nearer the surface. Before you decide, the questions people always ask
before consultation
It is fair to say the nose is an area that calls for caution. The vessels around it connect with those that supply the eye, so although it is rare, filler that blocks a vessel has been reported to cause skin necrosis, visual disturbance and even blindness. That is why we go over the anatomy first, inject small amounts slowly, and stop immediately to manage the situation at the first warning sign.
Yes — pain you can barely bear while the filler is going in, sudden pain around the eyes or across the forehead, vision that blurs or darkens, or skin that turns white. If even one of these appears, tell us at once. The sooner we know, the more room we have to act.
Previous surgery can leave the tissue adherent and shift the vessels away from where they usually run, so we look at such a nose far more carefully. Please be sure to tell us when the surgery was done, what was done, and whether an implant is in place. In some cases revision surgery, rather than filler, is the safer option and gives the better result.
Building up a low section of the bridge or softening a small irregularity is the realistic range. Projecting the tip or narrowing the nostrils is difficult to do safely with filler, and the tip in particular is a spot where the blood supply is delicate. If the change you have in mind belongs to surgery, we will tell you so.
If more filler is layered on before the previous material has been absorbed, it can spread sideways and make the bridge look broader. That is why we check how much is still there and, when needed, dissolve it first and rebuild the shape from scratch. Telling us when you had filler and how much helps us make that judgement.
